I log in, and I am no longer locked, and/or some chains have gone invisible.

Most probably your HUD or toy has lost its settings when you crashed or lost connection.

To fix, you need to do this:

• relock yourself.
• rez a new set of cuffs for the chains to re-chain themselves.
  
• perform the Save your Settings routine.
